A correction concerning the spelling of my father’s surname
In connection with the publication of the list of missing persons I would kindly request that you correct the spelling of my father’s surname, [published] in “Zorza” no. 16 of 16 April 1989, item 2648 – it is stated therein as follows: Lieutenant of the Infantry (Reserve), Tomasz Adam Machczyński (1 January 1899), an officer of Corps District Command [no.] VII. Whereas his surname should be spelt thus: Machciński.
The following information is provided as per the instruction:
1. Tomasz Adam Machciński, parents’ names: Jan and Maria née Bulińska, born on 1
January 1899 in Czermin, district of Rypin, Pomeranian Voivodeship, until 1939 resident in Poznań [at] Jackowskiego Street, from where he was called up to the Army in Baranowice [sic!].
| 2. | Secondary education, a professional officer of the 57. Infantry Regiment. Married to Stefania née Kubicka, with whom he had two children: Andrzej and Irena. |

7. Attached please find a [photo]copy of the letter and envelope, and also of the document recognizing him as deceased. I can send you my father’s photograph at a later date, for I have sent it in for a reproduction to be made.
8. Daughter – Irena Jackiewicz, resident together with my mother, Stefania, the wife of my father, Tomasz.
My mother is currently 88 years old and continues to be affected by her husband’s loss, she basically lives in the past. Thus I am writing to you, Dear Sir, with a request that you make it possible for us to purchase the book which you intend to publish.
My father’s photograph is not yet ready, so if you would like me to send it, then please provide an address, so that we get everything tied up.
Father is not mentioned in Andrzej Leszek Szcześniak’s “Katyń. Lista ofiar i zaginionych jeńców obozów [Kozielsk, Ostaszków, Starobielsk]”.

The objective of the questionnaire form/biographical note is to gather information about Poles who were murdered on the basis of the decision passed by the Political Bureau of the Central Committee of the All-Union Communist Party (Bolsheviks) on 5 March 1940.
Additional pages with notes concerning individual points may be attached to the questionnaire form. The questionnaire form is to be completed in capital letters.
I. Basic personal data of the murdered person

2. Exact date (day, month, year) and place (city, district, commune, former voivodeship) of birth:

Bulińska.
5. Education, profession, place of work before the start of the War in 1939:
A graduate of the Infantry Officer Cadet School in Warsaw, promoted to Lieutenant in 1923. In the 1920s he held various positions in the 62. and 57. Infantry Regiments. Until June 1939 he was the co-proprietor of the WIMA transport company at Długa Street, Poznań.
6. Place of residence – the address of the murdered person before the start of the War in 1939:
Poznań, Jackowskiego Street.
7. Military rank, position held when the War broke out in 1939:
A Lieutenant of the Reserve since 1929.
8. Military assignment, point of concentration.

9. Did he correspond with his family? Please provide the dates of written correspondence or photocopies or originals of letters.
He sent correspondence, and the last letter was dated 24 November 1939 – attached please find a [photo]copy.

Yes, no evidence attesting to persecution has survived.
12. Other important data concerning the murdered person:
Called up to the Polish Army in July 1939 and posted to the 78. Infantry Regiment in Baranowicze in the rank of Lieutenant.
