In Słupia Nowa on this day, 5 May 1948, at 12.00 p.m., I, Roman Zychowski from the Citizens’ Militia Station in Słupia Nowa, acting on the basis of the following: Article 20 of the provisions introducing the Code of Criminal Procedure, on the instruction of the Prosecutor of the District Court in Kielce, file no. ŻN 64/47, Article 257 of the Code of Criminal Procedure, due to the unavailability of a judge in the township, in consequence whereof any delay could result in the disappearance of traces or evidence of a crime, which traces or evidence would cease to exist before the arrival of a judge, observing the formal requirements set forward in Articles 235–240, 258 and 259 of the Code of Criminal Procedure, with the participation of reporter Stanisław Tomczyk, whom I informed of his obligation to attest to the conformity of the report with the actual course of the procedure by his own signature, have heard the person named below as a witness. The witness, having been advised of the importance of the oath, swore the requisite oath, and was also notified of the right to refuse testimony for the reasons set forward in Article 104 of the Code of Criminal Procedure, and of the criminal liability for making false declarations, this pursuant to the provisions of Article 140 of the Penal Code, thereupon stating:

As regards the present case, I | am aware of the following facts: On 1 May 1943 German |
gendarmes drove up to the village of Jeziórko, commune of Słupia Nowa, and carried out a sweep operation during which all the menfolk were caught. Among others, they then shot dead the following: Józef Klusek, Stanisław Wrona, Idzi Klusek, Wojciech Grad, Józef Rak, Stanisław Dadziak, Fr. Starnowski, Antoni Marzec, Stefan Ostrowski, Michał Sakowski, and Bolesław Góźdź. All of those mentioned above were buried in the cemetery in Dębno, commune of Słupia Nowa. The execution was the result of a denunciation made by one Józef Klusek, who had informed the gendarmes stationed in Słupia Nowa that there were local bandits in the village. Just before the execution, Stanisław Wrona tried to convince the court martial that since he worked together with Witek, the chief German informer, he should not be shot. Also Idzi Klusek explained that he was the brother of the village leader, Józef Klusek, and that his brother knew that he was innocent. This got them nowhere, however, and the sentence was carried out. I do not know the surnames of the perpetrators of the crime, for they were strangers, and had come to our village only to perform the execution.
At this point the report was brought to a close and read out before being signed by the witness.
