Warsaw, 28 March 1947, at 4 p.m. I, the abovementioned [?] corporal Marian Lenartowicz, from the XVI Citizens’ Militia Station in Warsaw, in the presence of the following witnesses:

whom I advised regarding their obligation to attest to the conformity of the report with the actual course of the procedure by their own signatures, interviewed the person named below as a witness. Having been duly advised, the witness declared as follows:

As regards the present case, I am aware of the following facts. I have been living in the house at Olesińska Street 5 since 1935. During the Warsaw Uprising the Germans mined our house. My family and I managed to escape before the explosion, however a considerable number of people remained in the house; the detonation resulted in their deaths, as they were buried under the ruins of the building.
Recently, on 25 March 1947, workers performing repairs on the house noticed human bodies – remaining from the Warsaw Uprising – lying in the rubble. They notified the Citizens’ Militia of their discovery.
These are all the facts known to me in the case.
At this point the report was brought to a close and, after being read out to the witness, duly signed.
